Dentist are often faced with challenging cases on a daily basis. Sometimes the situation is so tricky that as a doctor we wish to help out our patients in all situations but the technicality of the damage may be a big limiting factor. One such testing situation is that of lost crown portion of the tooth with only 2-3 mm of coronal structure remaining along with the complete root.
As a passionate doctors we would never want to deprive our patients from any opportunity that is manageable so as to save the tooth and restore the smile back .
We believe in Two things , the first is respect the nature by restoring a damaged tooth to its full potential if possible, second to be minimalist in our approach as we believe that there is always a next time for a more aggressive solution till we exhaust our restorative possibilities.
CLINICAL CASE
This article also discusses one such situation wherein a young men suddenly broke his previously capped front tooth and it was an aesthetic and functional emergency. The person concerned was about to get married in a week’s time too.
After detailed clinical, radiographic and photographic examination it was realised the remaining structure was healthy, the roots were previously treated for Root canals and what was noticeable was that there was a fractured fiber post in the root canal that was holding the broken cap which was now a technical challenge.
We decided to help our patient with minimal invasion and keeping the economics affordable. We used Waterlase YSGG laser at identity plus dental centre Gurugram to loosen the fractured fiber post . Waterlase helps to thermally melt the resin cement and allows removing metal free crowns, zirconia restorations and veneers by debonding without damage to tooth structure and the restorations in majority of the cases.
Once the Fractured post was out , we decontaminated the root canal under rubber dam and with waterless laser, subsequently a new fiber post was bonded and core build up done with fiber ribbon and composites. Interlig fiber strip was used to create the palatal wall and acted as a strong reinforcement. Finally the full crown was
created with direct resin composites free hand layered , it was finished for proper shape and anatomy . Final polish was done with polishing paste and the end result was so natural that the patient was almost in tears.
